Transforming Care Programme
As a specialist care provider, Godfrey Care passionately supports the Transforming Care Programme (TCP) and the national agenda around “Building the right support”.
We work with commissioners from local authority and ICB’s to understand their market position, and develop bespoke service models to support the implementation of the national service model for people with a Learning Disability and/or Autism who display behaviour that challenges, including those with an associated mental health condition. We offer TCP placements for individuals across both our Residential and Supported Living services.
As an experienced Provider, we recognise that not everybody needs an ordinary home, some people need an extraordinary home. A place that can meet their individual needs. We consider the multiple challenges and barriers that people with a Learning Disability and/or Autism face, and the inequalities that prevent them from accessing quality community services that understand their needs, which can lead to missed opportunities and inappropriate hospital admission. We know that for individuals with a forensic history, or those that have been detained due to a breakdown in placements, or mental ill-health living in a hospital setting that discharge without a suitable Provider can be a extremely challenging.
We actively work with partners, including commissioners, housing associations and community health practitioners to support some of the most complex individuals in society to have access to high quality housing and bespoke care and support. With our placement offer, we can ensure that individuals have robust physical environments designed to support their challenges to environments, as well as being supported by our highly skilled and experienced staff teams, with interventions designed by our clinical team in consort with people’s health teams.
With quality of life at the heart of everything we do, we develop the specific placement type individuals require, and then formulate a coordinated plan for a specialist assessment, transition and move from hospital. With our skilled teams experienced in working with individuals subject to Mental Health Act, Mental Capacity Act or Ministry of Justice restrictions, we prevent the replication of hospital settings while ensuring that individuals have the support they require for the long term.
Autism
We appreciate that each person on the Autism Spectrum has different needs and the training delivered through our partnership with NAPPI UK ensures staff are aware of the difficulties faced by individuals on the Autism Spectrum and more importantly how to effectively support these individuals in a positive and person centred way.
Epilepsy
Godfrey Care have ensured that staff are trained to effectively support individuals who live with Epilepsy. Training delivered by NAPPI UK ensures that staff are given the underpinning knowledge required to recognise different seizure types, identify triggers to seizures and to know how to support a person before, during and after a seizure.
